At Hospice of the Valley, we continue to expand to meet the needs of our community. Our full-spectrum care goes far beyond helping those at end of life. Today, we offer home-based primary care to manage acute and chronic illness and improve quality of life. We also have Palliative Home Care, providing education, support, and coordination of care to maximize symptom management and access to community resources. Palliative Care for Dementia works with patients and families in meeting the challenges of living well with dementia. The Campus for Dementia Care and Education is set to open in 2021. This unique community resource will focus on helping those at all stages and with all types of dementia from the time of diagnosis. Our skilled team also provides support and education to overwhelmed family caregivers.
