Community Hospice & Palliative Care has been around for over 40 years, but is only about 3 years old in the Suwannee County area. Our non-profit organization was started by a Pastor and a Social Worker who worked together to care for an elderly patient in their congregation. We provide end of life care wherever you call home. Home can be at home, in a facility, or under the I-75 overpass. We meet you where you are.
Hospice services are FREE to ALL who need our help. No one ever pays a dime or is turned away.
Community Hospice & Palliative Care has 10 in-patient units, however none of them are in the Suwannee County area. Our solution to this is to provide 24/7 care in the home, instead of moving the patient to a different location.
Hospice services are not for the dying. They are for those who want to live their most comfortable life for the remainder of their life. We have the ability to control pain and symptoms of a disease, to educate the family, to provide a tremendous amount of support and assistance to families and patients, and to help care for patients by bathing and providing personal care for them. Community Hospice & Palliative Care consistently scores higher than our competitors on patient care satisfaction surveys - which means that we do it better than the rest!
Anyone who needs help caring for a loved one should call us and ask for help. We want the "H" word (most are afraid of the word hospice) to be HELP! We can help! It's free to try. If you don't like our services, we go away. If you change your mind, we can come right back! Do you know anyone who needs HELP today? Call Chad Thrift at 904-200-0240 or Wendy Crews at 352-213-8713 for more information. Our TEAM is ready to HELP right now!
